About the Program

What Is the Social Care Network?

The Social Care Network (SCN) is a statewide initiative in New York designed to bridge the gap between the healthcare system and the social services communities need to thrive. Recognizing that health outcomes are deeply shaped by social conditions — where people live, what they eat, how they get to appointments, whether they can pay rent — the SCN creates structured pathways to address these health-related social needs.

ASHO is a certified SCN partner, bringing decades of bilingual community outreach experience to two distinct SCN initiatives across New York City. Through the SCN, ASHO navigators meet community members where they are — in neighborhoods, community centers, and homes — conducting screenings, providing personalized navigation, and connecting individuals and families to services that address their most pressing unmet social needs.

Our Process

How the Social Care Network Works

From first contact to ongoing support — here is how ASHO walks with every community member through the SCN journey.

  1. Community Outreach

ASHO staff conduct outreach across neighborhoods, in English and Bangla, connecting with families who may be unaware of available social services.

  2. Needs Screening

A brief, private screening identifies each person's social needs — from food security and housing to healthcare access and employment support.

  3. Service Connection

ASHO navigators connect individuals to the appropriate services and programs based on their unique needs — both within ASHO and through partner organizations.

  4. Follow-Up & Support

We follow up to ensure services were received and continue support as needs evolve over time.
